The 4 Cs: Interviewing Patients
in the Heat of the Moment
As healthcare providers we must be aware of and accurately assess the
climate-culture of our patients, as it will influence how we interact with
them.
One way to accomplish this is by using the 4 C's - a straightforward way for clinicians to understand the
perspective of patients who are not from the clinician's culture.
Using the 4 C's in discussions with patients
can help providers communicate more effectively and enhance the
provider-patient relationship.
1) "What do you CALL your problem? "
This is
one way of asking "What do YOU think is wrong?" It's getting at the patient's
perception of the problem. Remember some symptoms have very different meanings
in different cultures.
2) "What do you think CAUSED
your problem?"
This
gets at the patient's beliefs regarding the source of the problem. Some
cultures believe that disease is caused by an upset in body balance, a breach of
taboo, punishment from God or spirit possession. The perceived cause may need
to be treated as well.
3) "How do you COPE with your
condition?"
This
can also be asked in terms of, "What have you done to try to make it better:
who else have you been to for treatment?" Some cultures promote traditional,
alternative healers or home remedies.
It's important that healthcare
providers learn to ask for this information in a nonjudgmental way.
4) "What are your CONCERNS
regarding the condition?"
This should address underlying questions such as, "How serious do you
think this is?" "What potential complications do you fear?" "How does it
interfere with your life, or your ability to function?" Understanding the
patient's perceptions and fears regarding their condition and concerns about
any prescribed treatments are crucial.
